Q:- Which type of services provided by the Electoral Department?
Ans: Electoral department undertakes the work relating to preparation/revision of electoral rolls, preparation/issuance of photo identity card to all eligible electors in the State, conduct of Election, bye-election for "Presidential/vice Presidential" post and Parliament and State Legislative Assembly. These works are one under the direct control, superintendence and direction of the Election Commission of India.
   
 
  Electoral Rolls are prepared/revised in a manner prescribed by Election Commission of India with reference to the qualifying date.
Q:- Relevant form(s) the user has to file in order to avail this scheme/service?
Ans: Relevant forms are mentioned in Registration of Elector Rules,1960 which are 10 in number. A set of the forms is enclosed. Brief description of the forms pertaining to the public is discussed as under:-
 
Form No.1. Description Statement as to place of ordinary residence by a person holding a declared office.  
Form No.2.

Statement as to place of ordinary residence by a member of the Armed Forces.

 
Form No.2A.

Statement as to place of ordinary residence by a member of an Armed Police Force of a State, who is serving outside that State.

 
Form No.3.

Statement as to place of ordinary residence by a person employed under the Government of India in a part out side India.

Letter of request.

Form No.6. Claim application for inclusion of name.

Objection of inclusion of name.

Form No.8 Objection to particular in an entry.

Q:- Where to go (who accepts the application designation)?
Ans: Relevant forms can be obtained from and submitted to the designated officers at designated places, concerned Electoral Registration Officer, Assistant Electoral Registration Officer (Details given at annexure A&B).
Q:- Timing (For submission of the forms)?
Ans: During officer hour or any working day.
Q:- Whom to meet (Designation of the officers)?
Ans: Intended person can meet to the concerned ERO/AERO or concerned ET/ENT at the District Election Office.
Q:- Fee for forms and for the service?
Ans: No fee is charged for any form. However following fee is charged for the service mentioned against each:-
  1. For inspection of Electoral Roll                                     Rs.12/-
  2. Copying fee                                                               Rs.12/-
  3. Fee for urgent copy                                                    Rs.12/-
Q:- Check list and eligibility criteria?
Ans: Action with regard to additional/deletion in the Electoral Roll is taken by the ERO keeping in view the nature of revision. Person who files his claim in from 6 or his eligibility is objected to by someone else, should bring his matriculation certificate, ration card or other proof of age/residence.
Q:- When and how may days the user should receive response and condition if any?
Ans: Electoral Registration Officer is required to take action on the request of claim and objection in a time bound manner as prescribed by Election Commission of India and Representation of People Act, 1950.
 
  Procedure for obtaining photo identity card
  Every elector whose name is enlisted in the voter list is entitled to get his photo identity card.
Q:- Relevant Form(s) the user has to fill in order to avail this scheme/service?
Ans:
For Elector Photo Identity Card FORM I/D ECI EPIC-001
For Duplicate EPIC FORM I/D ECI EPIC-002
For Rectification of defects in EPIC and issue of duplicate cards FORM I/D ECI EPIC-003
For rectification of defects in EPIC FORM I/D ECI EPIC-004
Q:- Where to go (who accepts the application)?
Ans: Form can be obtained and submitted in the office of concerned Electoral Registration Officer or Concerned District Election Office in normal course or designated officer at designated places announced by District Election Officer.
Q:- Timing (For submission of the forms)?
Ans: During Office hour on any working day or specified by the District Election Officer.
Q:- Whom to meet (Designation of the officers)?
Ans: Concerned Electoral Registration Officer/Assistant Electoral Registration Officer/District Election Officer/Election Tehsildar/Election Naib Tehsildar.
Q:- Fee for forms and fees of the service?
Ans: No Fee is charged for forms. No charges for the EPIC issued for the first time. Rs.25/- charged for duplicate card, if earlier card is lost or mutilated.
Q:- Eligibility criteria for EPIC?
Ans A person should be enrolled in the part/village/town of Electoral Roll of which he needs EPIC.
Q:- When will and how many days the user should receive response and condition if any?
Ans: On the same day in case of on-line production of EPIC and within 1 or 2 days in case of off-line.
